A disciplinary record refers to disciplinary matters or accountability actions such as, disciplinary probation, suspension and/or expulsion for violations of the CSUB Student Conduct Code. Please complete and submit this request:
- If an institution, employer, agency, or investigators require a disciplinary standing letter from CSU, Bakersfield to determine if you are in good disciplinary standing with the university
OR
- If our office needs to complete and sign a Disciplinary Record Form from an institution, employer, agency, or investigators. Upload the document(s) along with the *Release of Information Authorization,, or comparable Release of Information form signed by the current/former student and submit this form.
*Release of Information Authorization is required by our institution and we will not release or process any disciplinary record request without an authorization to release information of a current/former students disciplinary record.
Our office will process your request once it is received by our office.
Please note, our office does not process academic standing related verifications to determine you are in good academic standing with the university, please contact the Office of CSUB Registrar for academic standing related inquiries.
