A disciplinary record refers to disciplinary matters or accountability actions such as, disciplinary probation, suspension and/or expulsion for violations of the CSUB Student Conduct Code. Please complete and submit this request:
- If an institution, employer, agency, or investigators require a disciplinary standing letter from CSU, Bakersfield to determine if you are in good disciplinary standing with the university
OR
- If our office needs to complete and sign a Disciplinary Record Form from an institution, employer, agency, or investigators. Email the document(s) along with the *Release of Information Authorization,, or comparable Release of Information form signed by the current/former student and submit this form.
*Release of Information Authorization is required by our institution and we will not release or process any disciplinary record request without an authorization to release information of a current/former students disciplinary record.
