Addendum for Lost/Stolen Reports

Who: Staff or Faculty

When: Use this form to report the loss or theft of a campus owned workstation or laptop that contained Level 1 confidential data.

How: Complete and sign the form. Attached the signed form to the completed Equipment Loss/Stolen/Destroyed Reports form. Send the completed forms to the University Police Department.  Send a copy of the forms to the CSUB Information Security Officer. Keep a copy of the forms for your department.

Equipment Loss/Stolen/Destroyed Report

Who: Staff or Faculty

When: Use this form to report the loss, theft, or destruction of a campus owned equipment.

How: Complete and sign the form. Attached the signed form to the completed Equipment Loss/Stolen/Destroyed Reports form. Send the completed forms to the University Police Department.  Keep a copy of the forms for your department.

Inventory Equipment or Materials Check-Out

Who: Staff or Faculty

When: Use this form when Faculty or Staff borrow campus-owned equipment and materials.

How: Complete the form. Make 2 copies of the completed form. Give one copy to the borrower. Keep one copy with the loaning department.  Send the original form to Property Management.

Who: Staff or Faculty

When: Use this form to report the disposition of lost, stolen, or destroyed property and to request authorization to dispose of State property.
