Renewals - Continuing Projects

If you wish to continue your project, please submit the Renewal Request through Cayuse HSIRB.

You are responsible for renewing your protocol annually. The renewal must be approved by our office before the day of expiration.

Please allow one month for renewal processing: our office may request clarifications or revisions that must be addressed by the researcher(s) before renewal can be approved.

Renewals will not undergo pre-review until all pertinent documents have been received.

Closures

A protocol can be closed when data collection activities including participant recruitment, enrollment, interventions, interactions, and follow-up has been completed. Principal Investigators may continue with data analysis and dissemination of findings.

If you wish to closeout your project and submit your Final Progress Report, please complete and submit the Closeout Request through Cayuse HSIRB.

Modifications

  4. Professional Qualifications
  5. Purpose
  6. Research Assistants and/or Key Personnel
  7. Sponsor/Funding Source
  8. Subjects (including adverse reactions, number of participants or risks)
