Dental and Vision Benefits

DENTAL BENEFITS
The University offers two types of dental plans, a "fee-for-service" plan and a managed care plan. Currently, there is no monthly premium cost to the employee for either plan. Website links to the listing of providers and claim forms are included below each plan description.

Fee-For-Service Dental Plan
This plan allows you to choose your own dentist and you pay a percentage of the charge based on the type of services performed. This plan is also subject to a deductible which must be paid prior to some services being performed. Currently, the CSU offers Delta Dental of California (Delta Premier) as the "fee-for-service" plan.

Managed Care Plan
 
This dental plan requires you to select from a network of contracted participating dentists. If you require specialty care, your primary dentist will refer you to a dental specialist who is also a part of the group. There is no deductible for this plan and low co-payments are required for those services not covered at 100%. Currently, the CSU offers DeltaCare USA as the managed care plan.

VISION BENEFITS
There is currently one vision plan provided at no cost to the employee, Vision Service Plan (VSP).   A website link to the list of participating providers is included below.  In order to recieve the greatest benefits under the plan, employees should register as a member on the VSP website, enabling employees and dependents the ability to access the physicians and optometrists contracted through the Select Network, our preferred vision provider network. 

When registering at the VSP website, the first screen asks for the last four digits of the SSN or full member id (SSN), first, last name and birth date.  The next screen instructs the employee to contact customer service or type in the full SSN. If the employee inputs the SSN (which is encrypted), another screen appears that asks for additional information.

Of the information requested on the screen, only the following items are required:
Log-on name
Password
E-mail address
Password question in case the password is lost
Birth date

Fields such as address, family member info, etc. are optional fields. Employees are not required to complete this information to obtain registration confirmation.

VSP is nationwide and therefore has several different provider networks. Creating a log-on allows the employee(s) to solely view providers that are a part of the VSP Select Network, which has been assigned to the CSU. Employees who wish to view a list of doctors in his/her area, but do not wish to sign-on, can still do so by answering “no” to the following question that appears on the “Find a VSP Doctor” screen: “Are you a VSP member?” Once the employee selects “No,” the following pop-up window should appear: “Because VSP offers multiple doctor networks, if you are not a member, or you do not sign on, we cannot guarantee that the doctors on the list participate in your plan.” If an employee downloads a provider list from the VSP website without signing on, the employee(s) will have to confirm VSP Select Network participation with the chosen provider or refer to the VSP Select Network provider list provided in Human Resources.

The plan allows you to receive routine vision care services and is not for the treatment of illness or diseases of the eye. You are eligible to receive one eye examination every calendar year. Lenses or contact lenses are provided every 24 months unless there is a significant change in the prescription. Frames are covered every 24 months. There is a co-payment required at the time services are provided. A supplemental Video Display Terminal (VDT) Plan is also available for those employees who meet CSU's eligibility requirements.  The VDT plan requires a claim form which must be taken to a participating provider to receive services.
