How to print a Trust Agreement Request/Update form
1. Type the Project Title or Name of the account
2. Type account number for existing accounts
3. Continue through all essential facts until all information has been entered. *It is important that you be complete in filling out the Source of Funds and the Types of Expenditures.
4. Expected duration of account must be completed, even if it is indefinite.
5. Disposition of funds at Termination of project must also be completed.
6. Please type the name of each person authorized to sign on this account and have form signed by signers. (Minimum of 2 signers).
7. PLEASE FILLOUT AND ATTACH A COPY OF THE CHARTFIELD REQUEST FORM.
Project Coordinator signature and Dean/Administrator signatures required prior to forwarding form to Fiscal and Support Services. A copy of the approved Trust form and Account number assigned will be forwarded to Department
