Admitted Students: RN to BSN

Once admitted to the program, RN to BSN students must meet the following requirements and upload to their Verified Credentials account (details to be provided in the acceptance letter).

Required RSVP

To reserve your seat or decline acceptance, the RSVP Form must be complete and emailed directly to oribaya@csub.edu. This is to be received no later than the deadline that is indicated on your conditional acceptance notice.

  • Please utilize the RSVP form provided in your acceptance email. 

Post-Acceptance Requirements 

You are required to complete a Background Check, Drug Screen and other requirements to succesfully hold your seat in the program. The Academic Advisor will be in contact with you regarding these additional requirements, once your RSVP Form has been recieved. 

Additional Instructions for completing requirements on Verified Credentials:

1) Physical Exam and Immunization Reference Sheet - Updated Resource Sheet 

2) Annual Safety Form - Review the CSUB Local Health Facility orientation, indicate the date you reviewed each topic on the form, then submit the form to Verified 

3) Clinical Agreement Form - Review the below documents for each topic, initial on the Clinical Agreement form that you have read them, then submit the Clinical Agreement Form to Verified

A note about Professional Liability Insurance: 

You will need to purchase Professional Liability Insurance through MyCSUB.  Please see the instructions below for more details. 

  1. When you log into MyCSUB, you will see your student portal.
    1. Under the Finances Tab, click on Account Inquiry.
    2. Click on Make a Payment. You will see the list of items that you can purchase.
    3. Click on Professional Liability Insurance Details.
    4. Click on Add to Basket
    5. Click on Checkout

RN-BSN Program Orientation 

All Accepted students must attend the Mandatory Online RN-BSN Program Orientation. The date of the orientation can be found in your letter of acceptance. Your Academic Advisor will provide you with further information once your RSVP Form has been recieved.
