Sample Contract for AMP Enrollees

AMP is an NSF/CSU program to increase the number of African-American, Hispanic or Latino-American, American Indian (Native Americans) and Pacific Islanders who graduate with baccalaureate degrees in Science, Mathematics, Engineering or Technology.  If you are a member of one (or more) of these targeted groups, please complete this form.

            1)   I wish to participate in the AMP program on my home campus.

                  By agreeing to this, I am authorizing AMP access to my grades.

            2)   I wish to be considered for internships identified by the AMP program.

            3)   I will attend the appropriate Academic Year Workshops sponsored by AMP and wish to be considered for AMP incentive awards in keeping with the regulations and criteria specified by AMP, i.e., regular attendance and participation in an Academic Year Workshop and earning a “B” or better in the related course taken in the same semester.

            4)   I agree to have my name and home or mailing address listed in a national directory of minority graduates in a SEM or Technology field during the year in which I expect to graduate.  Also, I authorize the use of my photograph to be used in AMP publications.
