PCI Self-Identification Form

To comply with Payment Card Industry (PCI) standards, the campus must identify all entities that accept payment via credit card and may need to perform an annual self-assessment. This applies to stateside and auxiliary departments as well as vendors who do business on campus.

A form has been developed to help the campus collect the appropriate information. The form needs to be filled out for the following situations:

  • If you directly accept credit cards.
  • If you use a vendor that accepts credit cards on your behalf.
  • If you want to receive monies via credit cards.

Once your response is received and reviewed, a member of Fiscal Services and/or Information Technology Services team will contact you about any next steps. Thank you for your assistance!
