Request for Reduced Course Load Form   

Used to Request a reduced course load when you encounter unexpected situation that may prevent you from fulltime enrollment during the course of study.

F-1 Transfer Eligibility Form

If you are intending to transfer to another educational institution, you must complete this form to facilitate the transfer of your immigration record in the federal database (SEVIS).

Rec-Biodata Revision Request Form

Used to submit to the ISP any changes in biographical data (such as legal name change, address change, etc).

Request for Immigration Document Form  

Used to request an I-20 for the student’s dependents (spouse and children).

Practical Training Application  (PTA) Form

Used to request employment authorization for practical training in the student’s field of study, either Curriculum Training CPT), or Pre-, Post-completion Practical Training (OPT) from ISP.

I-765 Form

Used to request employment authorization for practical training in the student’s field of study, either pre or post-completion (OPT) from USCIS.

I-539 Form

Used to request change of status (such as from B1/B2 to F-1 or from F1 to H-1). 

I-102 Form

Used to request for a replacement for a lost or stolen I-94 Card.
