Request for Exemption from Full Review
Instructions
Prepare and file the following packet of materials:
a. The cover page: "Request for Exemption by the IRB,"
b. Copy of any research insturments to be used: e.g., surveys, interview guides,
self-report scales,
c. Consent form, if necessary.
Protocols reviewed for exemption constitute human subjects research and are fully within the purview of the CSUB IRB. "Exemption" refers to exemption from full IRB review. Complete and print the Exemption Cover Page, obtain the requested signatures, and attach this form to the other materials. File your materials with the Office of Graduate Studies and Research [DDH D-108, (661) 664-2231].
Protocols for Exemption from full review may be submitted at any time and are generally "turned around" in one week. It is possible that a higher level of review may be necessary. It is strongly advised to consult informally in advance with the RERC.
