2. Informed Consent Form

	Your participation in this project is voluntary. If at any time you wish not to participate in this study, simply exit the survey. If you decide to complete the survey later, the program will return to the last question you completed (if you use the same computer). You may leave the study at any time, for any reason, with no penalty. You may skip any question.

	The data that are collected from all of the subjects will be used by the Western Association of Schools and Colleges (WASC) Accreditation Planning Committee and the University to develop the Institutional Proposal for the University's accreditation process. Information will only be published in summary form in university publications or academic journal articles. Your identity will in no way be published or presented, however. All survey information will remain anonymous and will be kept in strictest confidence. Only summary information will be provided to the participants and the WASC Planning Committee. Only the principal investigator will have access to the original data. Any individual information collected will be stored in the CSUB Teaching and Learning Center.
