You must use Firefox, Internet Explorer will not submit a protocol. And if you use IE, you will receive an incorrect message, stating the protocol has been received.

I affirm that the dept chair or program director has reviewed this protocol; for a student protocol I affirm that I have reviewed and approved the content.

Under penalty of professional misconduct, I affirm that the statements in the protocol are accurate and true.

I acknowledge that animals used for research and teaching are beings to be treated with care and concern. To that end I have read and agree to abide by the CSUB Policies and Procedures regarding the care and use of animals. Additionally, this project will be conducted in accordance with the NIH Guide for the Care and Use of Laboratory Animals. I will abide by all federal, state, and local laws and regulations dealing with the use of animals in research and teaching.
