Billing Request Form

Who: Staff or Faculty

When: This form is used for on- or off-campus billing

How: Complete and print the form, acquire signatures, attach supporting documentation, and route the completed form to Accounts Payable (35 ADM).

Budget Transfer Form

Who: Staff or Faculty

When: The Budget Transfer Form is used to transfer budget funding from one department/account to another department/account within the same fund. You cannot use the Budget Transfer Form to transfer budget from one fund to different fund.

How: You will need to complete the form, acquire the appropriate signatures, and submit the completed form to the University Budget Office.

Cabinet Carry Forward Form

Who: Staff or Faculty

When: Carry Forward Funds are state funds that are carried over from the prior fiscal year and can be used for Temporary One-Time Funds. If you have any questions, please do not hesitate to contact the University Budget Office.

How: You will need to complete the form, acquire the appropriate signatures, and submit the completed form to the University Budget Office.

Campus Administrator Delegation Application

Who: Administrator

When: This form is used to request delegated purchasing authority from the President in accordance with Executive Order 775 and subject the guidelines in the Delegation of Purchasing Authority Handbook,  Procurement Credit Card Handbook, and Direct Pay Handbook.

How: Complete the application, obtain signature of the appropriate Cabinet Officer and forward to the Director of Procurement for final review and approval.

Campus Carry Forward Form

Who: Staff or Faculty

When: The Budget Transfer Form is used to transfer budget funding from one department/account to another department/account within the same fund. You cannot use the Budget Transfer Form to transfer budget from one fund to different fund.

Chartfield Request/Signature Authority Form

Who: Staff or Faculty

When: This form is used to add, modify, or inactivate chartfields and to add or delete signers for specific chartfields.

How: Complete and print the form. Acquire the appropriate signatures, and route the completed form to General Accounting.

Expenditure Transfer Form

Who: Staff or Faculty

When: This form is used to to credit or debit chartfields within the same business unit.

How: Complete, acquire appropriate signatures, and email to General Accounting at accounting@csub.edu.

Labor Cost Distribution

Who: Staff or Faculty

When: Use this form to make changes to positions in the Labor Cost Distribution system, such as adding, inactivating, reactivating, updating, reclassifying, or other types of positions changes.

How: Complete the form, acquire the appropriate signatures, and route the completed form to Human Resources.

Payroll Move Form

Who: Staff or Faculty

When: The Payroll Move Form is used when salary and benefit expenditures for an employee must be moved to the appropriate department. You must have access to PeopleSoft and the ability to login to Labor Cost Distribution (LCD). If you do not have access contact your administrator, or direct the payroll move to the appropriate authorized personnel.

How: You will need to complete the form, acquire the appropriate signatures, attach supporting documentation, and submit the completed form to the University Budget Office.

Trust Fund Agreement

Who: Staff or Faculty

When: This form is used to request a new trust fund or update an existing fund. This agreement must accompany the Chartfield Request form.

How: Complete and print the form, acquire signatures, and route the completed form, along with a completed Chartfield Request form to General Accounting.
