Application for Fee Waiver Course

Change in Fee Waiver Courses

The Change in Fee Waiver Course(s) form is used by all Fee Waiver participants when ADDING, DROPPING, or CHANGING approved Fee Waiver classes. For more information, please refer to the Fee Waiver Program website.

Career Development Plan

The form, Career Development Plan, is used to prepare your career development plan for participation in the Fee Waiver Program. In order for course(s) to be Career Development, an approved Career Development Plan must be on file in the Office of Human Resources. Dependent Fee Waiver Applications

The Dependent Fee Waiver Applications are used to transfer your fee waiver eligibility as provided in Coded Memorandum HR 2001-23 or the applicable Memorandum of Understanding to a spouse, domestic partner, or dependent child.
