Verification Forms
Requested information must be submitted no later than 30 days prior to the end of the enrollment period for which you are seeking financial aid. Incomplete or late submission of this information may jeopardize your financial aid.
You will need to check your myCSUB "to do" list for specific items you need to submit. We may need a variety of documentation, depending on your FAFSA information.
Agency Verification of Food Stamp Benefits
This form is used to verify the amount of public assistance program food stamps you, your spouse, or parents (if dependent) received in the previous calendar year.
This form is used to verify the amount of child support you, your spouse, or parents (if dependent) paid in the previous calendar year.
Parent Income Certification Form (for non-tax filers)
This form is used to verify income for parents of dependent students who reported that they will not file a Federal Income Tax Return.
Spouse Income Certification Form (for non-tax filers)
This form is used to verify income for a student's spouse who reported that they will not file a Federal Income Tax Return.
Student Income Certification Form (for non-tax filers)
This form is used to verify income for students who reported that they will not file a Federal Income Tax Return.
Verification of Dependency Form
this form is used for undergraduate or post-baccalaureate teacher credential students who are independent solely by virtue of being a foster youth, emancipated minor, or homeless, must use this worksheet to verify their situation.
Verification of Household Size / Number in College
This form is used to verify the number of dependents in the household, as well as the number enrolled at least half-time in college.
