2011-12 Additional and/or Untaxed Income: This form is required when the student and/or parent have reported additional and/or untaxed income.

2011-12 California High School Graduation Certification Form for the Cal Grant Entitlement Program: This form is used by prospective Cal Grant recipients to verify that they have graduated from high school, or have passed the State of California High School Proficiency Examination (CAHSEE), or have passed the General Education Development (GED) test.

2011-12 Atheltic-Grant-in-Aid Appeal Form: CSUB has a hearing process in place for student-athletes who would like to appeal a decision made to cancel or reduce their athletic grant-in-aid. This procedure was designed in accordance with NCAA guidelines. Student-athletes wishing to have such a hearing should complete and submit this form, along with any supporting documentation, to the Director of Office of Financial Aid & Scholarships.

2011-12 California Registered Domestic Partner - Supplement to the 2011-12 FAFSA: This form is used by prospective financial aid recipients who are, or their custodial parent is, registered with the California Secretary of State as a domestic partner and are applying for, or receiving, institutional aid from a California State University campus.

2011-12 Certification of US Citizenship: Form used to document a student's United States citizenship or immigration status.

2011-12 Cost of Attendance Adjustment: Form used to request an increase in the Cost of Attendance.

2011-12 CSUB Academic Scholarship Recommendation Form: Form used by transferring and continuing students to obtain recommendations for an academic scholarship. This form is to be used with the CSUB Academic Scholarship Application 2010-12. 2011-12 CSUB Freshman Scholarship Application: Form used by incoming freshmen graduating from a local CSUB service area high school to apply for scholarships at the California State University, Bakersfield. The application, along with two CSUB Freshman Scholarship Recommendation Forms must be completed and returned to the Division of Undergraduate Studies by April 1st. The University's minimum freshman scholarship criteria include:

    1. Minimum cumulative GPA of 3.25
    2. Must provide ACT/SAT scores.

2011-12 Dependent Student Verification Worksheet: This form is required of dependent aid applicants who are selected by the Federal processor for "verification." The completed form enables the school to compare information from your FAFSA with signed copies of your and your parent's Federal tax forms, or with W-2 forms or other financial documents.

2011-12 Direct PLUS Loan Addendum: This form is used to determine a parent's eligibility for a Direct PLUS loan.

2011-12 Financial Aid Applicant Narrative: Form used by students who wish to request a change in financial aid, or to provide additional clarifying information.

2011-12 Independent Student Verification Worksheet: This form is required of independent aid applicants who are selected by the Federal processor for "verification." The completed form enables the school to compare information from your FAFSA with signed copies of your (and your spouse's, if you are married) Federal tax forms, or with W-2 forms or other financial documents.

2011-12 Loan/Term Adjustment Form : Form used to reduce, increase, or decline a previously awarded loan. You can also use this form to notify the OFA&S that you will not be attending a specific term.

2011-12 Loss of Income : Form used to petition the Office of Financial Aid & Scholarships to re-evaluate the applicant's financial aid eligiblity.

2011-12 Parent Income Certification: Form used to document sources and amounts of money received by the parents of a dependent financial aid applicant. Also used to explain special circumstances concerning the family financial situation.

2011-12 Parent Investment, Business, Farm & Savings Form: Form used by the student's parents to document sources and amounts of investment, business, farm and/or savings assets.

2011-12 Online Entrance Counseling Instructions: Instructions created by the Office of Financial Aid & Scholarships to assist financial aid applicants with the Online Entrance Counseling process.

2011-12 Online Entrance Counseling Session: Continuing your education is a major investment for your future. If you decide to make loans a part of this investment, managing the debt you incur is a major responsibility. To ensure that you understand this responsibility and the obligation you are assuming, the federal government requires you to participate in loan counseling prior to receiving a Federal Direct Loan, if you have not previously received a Direct Loan, Federal Family Education Loan, or SLS loan.

2011-12 Satisfactory Academic Progress (SAP) Petition Form: Form used by the student, who has mitigating factors that have hindered their satisfactory academic progress toward graduation, including death of a relative, injury or illness of the student, or other special circumstances, the student may submit a written appeal, with supporting documentation, to the OFA&S. If the appeal is approved, the SAP quantitative standards may be temporarily waived to provide the student with the opportunity to reestablish SAP. Appeals are determined on a case-by-case basis. The criteria for approving an appeal will be based on the student's circumstance, documentation, and a reasonable expectation that the student can reestablish progress toward a degree objective, and regain academic standing consistent with the requirements for graduation.

2011-12 Student Consent Form to Release Information to a Designated Third Party: Form used by the student, to allow CSUB OFA&S to release financial aid information to a third party, such as; Parent, Spouse, Sibling, Friend, etc.

2011-12 Student Income Certification: Form used to document sources and amounts of money received by the student (spouse). Also used to explain how expenses were met when reported income was insufficient to pay for rent, food and other expenses.

2011-12 Student Investment, Business, Farm & Savings Form: Form used by student to document sources and amounts of investment, business, farm and/or savings assets.

2011-12 Summer Financial Aid Application: Form used to request aid for Summer financial aid.
2011-12 Transfer Entitlement Cal Grant Verification Form: The California Student Aid Commission requires the OFA&S to obtain documentation that verifies that the student has met specific eligibility criteria.

2011-12 VA Education Benefit Verification Worksheet: This form is required when the student has reported receiving VA Education Benefits on question #50 & 51 of the FAFSA.

IRS Form 4506: (Request for Copy of Transcript of Tax Form): Form used to obtain a copy or summary transcript of your prior year(s) Federal Tax Form 1040, 1040A, or 1040EZ. When applying for financial aid, if you have been selected for "verification" by the Federal processor, you will need to provide a copy of your (and your parent's, if you are dependent) latest Federal Income Tax Return or tax account information from that return. If you didn't keep one for your files you must complete and submit the IRS Form 4506.

IRS Publication 970 - Tax Benefits for Higher Education: This IRS publication explains the tax benefits for persons who are saving for or paying higher education costs for themselves and members of their families. It covers the following topics.

  • Two education credits, the Hope credit and the Lifetime Learning credit.Education individual retirement accounts (education IRA's).Withdrawals from traditional or Roth IRAs.Interest paid on certain student loans.The cancellation of certain student loans.Qualified state tuition programs.Interest earned on certain savings bonds.
  • Employer-provided educational assistance benefits.

Student Survey of Office of Financial Aid & Scholarships Services: Form used by anyone to survey responses, which are used to help the staff improve services provided to students. All surveys will remain completely confidential.

FAFSA on the Web: A free U.S. Department of Education web site where you can complete the Free Application for Federal Student Aid (FAFSA) online and submit it via the Internet. You can use FAFSA on the Web on a personal computer (PC) or a Macintosh that is equipped with a supported browser. A Personal Identification Number (PIN) is required to allow parents and students to electronically sign their electronic FAFSAs. Visit the Department of Education's web site for PIN Registration and select the link "Request a PIN (first time)." Ford Federal Direct Loan Program

Direct Loans Brochure: A brochure all about Direct Loans including types of loans, loan amounts, total limits, interest rates, how to apply, entrance counseling, loan payment, what happens while you are in school, repaying your loans, options for postponing repayment, forbearance, default, loan discharge, loan consolidation, borrower rights and responsibilities, and questions about your Direct Loan.

Direct PLUS Loan Basics: Information about the Federal Direct Loan including application procedures, eligibility, borrowing limits, interest rate, disbursement, repayment, deferment, forbearance, discharge/cancellation, consolidation, and questions about your Direct PLUS Loan.

Entrance Counseling for Borrowers: Provides direct access to the U.S. Department of Education's on-line entrance counseling session. The Federal government requires participation in loan counseling prior to receiving a Federal Direct Loan. Completion of this on-line entrance counseling session is one means of satisfying this requirement.

Entrance Counseling Guide for Borrowers: Provides basic facts about subsidized Federal Direct Stafford/Ford Loans (Direct Subsidized Loans) and Federal Direct Unsubsidized Stafford/Ford Loans (Direct Unsubsidized Loans) and explains rights and responsibilities as a borrower. Also contains useful tips to help develop a budget for managing educational expenses and financial resources as well as important information to help manage and repay student loans.

General Forbearance Request: Form used to request an arrangement to postpone or reduce the amount of a borrower's monthly Direct Loan payment for a limited and specific time period due to a temporary financial hardship.

In School Deferment Request: Form used to request deferment of loan payments while the student is attending school.
