Subject:   Mailing address W-2 / W-4

At the bottom your State Controller's Office pay stub for the 10/31/05 paydate you will find the address that your 2005 W-2 will be mailed to you. If you have moved and not notified Human Resources (HR), please do so by November 15th so that your W-2 will go to the correct address. The Office of Human Resources is located in Admin 104. NOTE: The State Controller will charge you $8.50 to replace a "lost in the mail" W-2. If you are a Foundation employee and need to complete a W-4 with your new address, you will also do that in H.R.
