Downloading/Printing Instructions

1. Go to the Forms page, and click on the link associated with the form you wish to download/print.
2. After the form/image has finished loading, make sure the frame in which the form is presented is selected (in other words, click on the frame in which the image or form is located).
3. For printable forms, simply click on the Print button located on the toolbar at the top of your browser.
4. For downloadable forms:
   • Click the "Download" link above the picture form to download the form to you computer.
   • Right-click on the image and select Save image as... to save the image to you computer.
