Application for Graduation
Application for Graduation (Grad Check) should be filed at least two quarters before their expected date of graduation. Complete the form as follows:
- Complete the application (name, address, term, etc.)
Pay the graduation filing fee at the Cashier's window or online. If paying online, attach a copy of the receipt or transaction once payment has been made.
Initial application fee: $55, Re-Application Fee: $55
- Return the completed form to the Evaluations office (inside Admissions and Records)
Once the application is received in our office and we receive the major and minor concentration outlines from your department, we will process the application and reply to you as soon as possible. Please
remember the response time depends on when we receive all documents from the departments.
